As you surmised intuitively, this is karmic in origin. This is from a past lifetime with [name withheld] in a relationship that was quite troubling and there were issues of abandonment. And so, she is reluctant to step forward into marriage here, and this is only simmering below the surface and not in her conscious thoughts. While she has some nervousness about the plans, it is flaring up into turmoil within her at times, and she has episodes of disagreements and arguments. She continues to return to him and that is because there is a strong love bond that has stood the test of time, and is the overriding consideration here for both parties on a soul level and also energetically at this time with their coming together. This does not prevent karmic rumblings from past lives where there were differences and difficulties and tragedies of one sort or another. That is always the price to be paid in renewing old acquaintanceships. There often is a mixed blessing and the history of the ups and downs carried with them, and this will be reawakened and will be humming, so to speak.
In this case, there is no reason for great concern as things will work out as foreseen and planned. And you have seen this intuitively as well, and been told this is a good pairing. The pain in the knee is transient and will improve on its own as the relationship comes to a deeper level of commitment. That is the cure here, not to avoid the relationship, but to carve it in stone, so to speak, through the taking of vows. As you are pondering this, you will hear as well an echo from us to not raise this as a conscious concern for either party, as their stress level would not be eased in knowing there is something real to worry about here when this is not the case, and in fact is the opposite.
