Can you help me understand what is most needed to help him? Is this a spirit attachment problem, strictly karmic, ET targeting, or a combination of influences?

As you saw intuitively, this is primarily a spirit attachment issue. They have managed to breach his defenses during a low moment and have invited their friends to the party, causing an ongoing siege. This is so typical of what they will do to undermine their host and, at a minimum, create a state of high chronic anxiety because there will be an inner war underway, not understood or perceived at the conscious level but only feeling emotional consequences of the attack. So, careful clearing is in order here. There are karmic issues reinforcing things for the worse as well, but it would be a good test to see what the protocols alone will do. There is always room for improvement as a follow-up, but the first step is to get those dark beings out of the picture, at least for now, and get some further protections in place, and the healing protocols will help in doing both.
